We currently have opportunities for Social Workers within our Adults Social Care team who are enthusiastic and passionate about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference and enabling them to increase their independence and have fulfilling lives, free from harm and abuse.
We currently have vacancies in the following teams:
- Social Worker - Adults Social Care – Hospital/Post Discharge Team - Full Time Permanent. £37,336 to £45,441 per annum to include increments for weekend working.
Social Worker – Adults Social Care – Reablement Team. Part Time, 22.5 hours per week. £35,745 - £43,421 per annum, pro rata.
Social Worker - Adults Social Care – Hospital/ Post Discharge. Full Time
It is a requirement for Social Workers working in the Hospital Team to work Monday to Sunday (5 days a week) on a rota basis and is currently operating a hybrid working model and are able to work from a number of bases.
As part of an experienced team, you will play a key role in shaping and delivering this service, providing a specialist social work service to Solihull residents who require support to leave a hospital setting or require long term planning after a hospital admission. The role is subject to a 7-day working contract which involves all team members being part of a 7-day working rota, working 5-7 days a week. This will be at a 1-6 weekends worked, minimum requirement and has an enhanced rate of pay attached to this requirement. Bank Holidays are also required to be worked at a maximum of 3 per annum.
Social Worker – Adults Social Care – Reablement Team. Part Time
This post will require working 3 days per week which can be flexible to suit the candidate and the service, between Monday to Friday to include a Monday as one working day. This post is based at The Bluebell Centre, Chelmsley Wood.
In this role you will be working with people who are in receipt of short term Reablement services – to promote their independence and wellbeing using a range of interventions, and to complete needs and carers assessments for anyone requiring longer term support. You will work alongside the team’s Occupational Therapists where required as well as supporting Reablement Facilitators in the team specifically in areas around Mental Capacity and Safeguarding.

We are looking for people who have:
- A professional Social Work qualification and be Social Work England registered or be in the final academic year of a professional Social Work qualification with pending Social Work England registration.
- Knowledge of safeguarding principles and relevant legislation.
- Experience of delivering person-centred and strengths-based working including assessments and reviews.
- Detailed knowledge of the care legislation including the continuing health care framework.
- Experience of assessment, the development and planning of care and of positive risk management.
- Effective communication skills and experience of collaborative working to build effective working relationships.
