An exciting opportunity has arisen to join the City Wide Multi-Disciplinary Team and the Constituency Teams based within various locations city wide. Successful candidates will work across busy multi-disciplinary teams to facilitate social work practices.
You will work as part of the community social work teams to deliver on person centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention to support, guidance and sign posting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014.
You will have demonstrable social work experience, the ability to work positively as part of a multi-disciplinary team, in partnership with other stakeholders, together with an interest in and skills to work within the specific constituency based or city-wide multi -disciplinary team.
Working within the legislative framework, you will carry out social work functions, utilising relevant legislation to complete conversations and provide support and guidance, drawing on sound inter-personal and communication skills. You will require the ability to produce reports and the resilience to deliver under pressure within required timeframes.
You must hold Social Work England registration.
