An exciting opportunity has arisen to join the Adult Hospital First Contact Team. The role also includes an element of Out of Hours shifts.

The Adult Hospital First Contact Team works with adults, who are currently inpatients within a Sheffield Teaching Hospital. We support with discharge planning, Care Act assessments, screening safeguarding concerns and an element of case management, to facilitate a safe discharge plan.

Ideally candidates will have experience of crisis management and multidisciplinary work. The role incorporates both discharge planning for patients in Sheffield Teaching Hospitals, and being part of the out of hours team- which provides a duty response for all of Adult Care and Wellbeing services, evenings and weekends. Relevant IT skills will be required but additional training will be provided for specific systems. It is essential that applicants for this role are car drivers, and have access to a vehicle for our of hours shifts. Assessment and duty experience would be an advantage, but a flexible approach is important. Safeguarding experience is essential. The ability to work to short time scales and problem solve is also essential. Role will include application of the Care Act 2014 and Mental Capacity Act 2005.

The team is based at the Northern General Hospital and can include working with health colleagues across all disciplines. The role is office based.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
