The service is designed to provide an integrated model of social care that promotes a positive experience for service users and carers across the whole service.
LPFT have a Section 75 agreement under the National Health Service Act (2006) where Lincolnshire County Council have devolved their assessment and care management duties to LPFT. Social work in the Trust is delivered through a Community Social Care Resource where social care staff are aligned to the integrated community teams but are managed and supervised from within the social care resource. Three distinct Social Work teams exist covering the East, West and South of the county respectively. Each team has a Team Manager and mangers are expected to work closely in order to provide a consistent service across the county.
This is a fixed term post for one year, supporting others within the team as well as the Senior Social Worker and Team Manager. You will carry out direct work with service users and colleagues within other teams within the Trust; promoting multi disciplinary working. The base is flexible within the East area.
The post is full time 37.5 hours per week and this role is suitable for Newly Qualified Social Workers (Band 5) as well as experienced Social Workers (Band 6).
This post of Social Worker is a key role in the Trust’s Community Social Care Resource providing assessment and care management duties under the care act, supervising qualified and unqualified staff, playing a role in community capacity building and supporting the Manager within the team.

Ensure evidence-based practice and research awareness is used as the foundation for the assessment, planning implementation and evaluation of nursing care.
To engage with service users and their families/carers, developing therapeutic relationships, and providing evidence-based, psychosocial interventions in a variety of settings'
Act as care co-ordinator for an agreed number of service users, adopting the team approach to provide interventions to other service users, families/carers as appropriate.
Assess complex health and well being needs, develop, monitor and review care plans to meet specific needs.
Undertake initial and ongoing risk assessments, utilising standardized tools, in order to develop accurate and appropriate risk management plans and intervention, highlighting service user strengths, and positive risk options.
Undertake or facilitate Carer's appraisal of need as appropriate.
Develop and co-ordinate integrated team working with team, and other agencies involved in the care of the individual, their families/carers.
Promote service user and carer involvement; ensuring choices are embedded into the philosophy of care.
Empower people to address specific needs in relation to health and well being, and enable individuals to become expert in managing their condition.
Ensure social work perspectives and values are supported and represented within the multidisciplinary approach.
Keep up to date with current practices and issues in Mental Health and Social Work.
The post holder will be expected to undertake Approved Mental Health Professional Training, if not already qualified as such.
