We currently have opportunities for Social Workers within our Adults Social Care team who are enthusiastic and passionate about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference and enabling them to increase their independence and have fulfilling lives, free from harm and abuse.
We currently have vacancies in the following teams:
- Social Worker - Adults Social Care – Hospital/Post Discharge Team - Full Time Permanent. £37,336 to £45,441 per annum to include increments for weekend working.
Social Worker – Adults Social Care – Reablement Team. Full time, 12 Fixed Term Contract. £35,745 - £43,421 per annum. Secondments will be considered for this post.
Social Worker - Adults Social Care – Hospital/ Post Discharge
It is a requirement for Social Workers working in the Hospital Team to work Monday to Sunday (5 days a week) on a rota basis and is currently operating a hybrid working model and are able to work from a number of bases.
As part of an experienced team, you will play a key role in shaping and delivering this service, providing a specialist social work service to Solihull residents who require support to leave a hospital setting or require long term planning after a hospital admission. The role is subject to a 7-day working contract which involves all team members being part of a 7-day working rota, working 5-7 days a week. This will be at a 1-6 weekends worked, minimum requirement and has an enhanced rate of pay attached to this requirement. Bank Holidays are also required to be worked at a maximum of 3 per annum.
Social Worker – Adults Social Care – Reablement /Strokes Project Team. Full time, 12 Fixed Term Contract.
This post will require working Mon- Friday 9-5pm and is based at The Bluebell Centre, Chelmsley Wood.
This is an exciting time to be part of a new health and social care multi- disciplinary team whom will work collaboratively to support Solihull residents who require stroke rehabilitation. Working with the Speech and Language Therapist, Occupational Therapist and Psychologist, you will support the person and their carer to have a positive rehabilitation experience, which will include ensuring that they have access to the right care in the right place at the right time. The post will require you to have excellent communication skills, as will be expected to liaise with several professionals across health and social care settings, as well provide regular written and verbal updates at multi-disciplinary meetings.

We are looking for people who have:
- A professional Social Work qualification and be Social Work England registered or be in the final academic year of a professional Social Work qualification with pending Social Work England registration.
- Knowledge of safeguarding principles and relevant legislation.
- Experience of delivering person-centred and strengths-based working including assessments and reviews.
- Detailed knowledge of the care legislation including the continuing health care framework.
- Experience of assessment, the development and planning of care and of positive risk management.
- Effective communication skills and experience of collaborative working to build effective working relationships.
