This is an opportunity to be part of an exciting new service within Adult Social Care and in partnership with Mental Health Services. Innovative times are happening and if you believe in a holistic approach to supporting individuals who are struggling with their mental health and would like to work closely with multiple partners and be involved in forming a brand-new service then this is the job for you.
We are creating a new integrated personalised care team (IPCT) Health Integrated Network Team (MINT) which will help people receive the care they need, quickly and close to home. The new model will bring together Primary and Secondary care, Local Authority and Voluntary sector services, into new combined Mental Health Teams. The social work role will involve working with professionals from different organisations, but you will also have strong links with the Adult Social Care and depending on the location, the community teams in north & west or south Bristol will provide you with support and regular supervision.
The team aim to support individuals with complex mental health needs, who can fall between the gaps of current pathways, making sure they can have the personalised support they need with services around them.
The social work role would be integral to providing an excellent service by ensuring quick access to personalised mental health and social care support for improved experiences and outcomes for our citizens.

Assessing the needs of service users and carers, you will plan, implement, review and evaluate outcomes. Working for and with individuals, you will take care to understand their unique circumstances and priorities. Championing the principles of choice and control, you’ll support people to make their own decisions. In collaboration with you, they will be empowered to have services organised and risks managed in ways that are right for them. Constantly maintaining your professional standards as our service continues to develop, you’ll keep a keen eye on changing needs. From developing and reviewing support plans and provision, to enabling the creative use of resources including assistive tech, you’ll provide social care that is fit for the future.
Registered with Social Work England, you’ll have a social work qualification plus significant post-qualifying experience, or you’ll be newly qualified and looking to gain invaluable experience with our teams in Bristol. Highly motivated, with lots of initiative, you’ll demonstrate the ability to adapt your interventions in response to all kinds of people in a range of working environments. Naturally collaborative, you’ll bring to the role a flair for building relationships and working in partnership.
