Your role will centre on the delivery of specialist mental health support to older adults with mental illness who are requiring treatment and care in the community.
There will also be a requirement to complete statutory social work duties on behalf of the local authority including the completion of Care Act Needs Assessments, Mental Capacity Act assessments and to provide social work support to people subject to the Court of Protection.
This will be in the form of direct work and care coordination with people on caseload, providing duty support to those under the team and contributing to multi-disciplinary team meetings both in house and with colleagues externally. As a social worker you will ensure this work is undertaken in a strengths focused manner and from a rights based perspective.
Empowering and supporting service users to make effective personal decisions which promote their psychological well being and recovery.
Undertakes supported person led assessments, risk assessments, support planning and outcome focused reviews of service users and carers who have eligible needs and complex and/or long term conditions in accordance with legislative requirements, policy and professional frameworks.
In addition to undertaking person led assessments, you will manage a caseload with complex needs and provide case load guidance, support and specialist advice to junior members of staff and ensure care is completed within the practice and performance framework.
