CYC Adult Social Care are looking to recruit 1 Social Workers (Hospital Social Work Team), this is a qualified position.
The role would require you to work within a fast paced environment, within a supportive Hospital Social Work Team facilitating timely and safe hospital discharges. The team work to facilitate rapid hospital discharge and also complete assessments post discharge in line with the Discharge to Assess Guidance. Regular supervision and training are provided. You will hold your own caseload working within a supportive team of qualified and unqualified workers.

If you are interested in applying for this role you will need a working knowledge of legislation relating to Adult Social Care, Safeguarding Adults and Mental Capacity. You will have excellent communication skills as well as skills in assessment, partnership working and accountable practice.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1: Can express themselves fluently and spontaneously, almost effortlessly. Only a conceptually difficult subject can hinder a natural, smooth flow of language.
